Does my coverage include a wellness visit?

Quantum and Epic plans include at least one preventive doctor office visit each coverage period. Economy, Choice, Standard and Deluxe plans do not cover preventive care so any wellness visits would be subject to the deductible. However, Choice and Deluxe plans have a doctor office visit copay. Quantum and Epic plans are not available in all states.
